- Receive respectful care and clear information about pharmacy services.
- Ask questions about prescriptions, directions, storage, expected effects, and important risks.
- Know the identity and role of pharmacy personnel involved in your care.
- Take part in decisions and discuss alternatives with your prescriber.
- Choose a pharmacy, subject to law, network rules, and service availability.
- Raise concerns free from retaliation.
- Receive communication assistance and accessible information when required.
2. Health-information rights
HIPAA and other applicable laws may provide rights to inspect or obtain copies of certain health records, request an amendment, request restrictions, ask for confidential communications, receive an accounting of certain disclosures, and obtain a paper copy of the Notice of Privacy Practices. 3. Your responsibilities
- Provide accurate information about identity, allergies, health conditions, medicines, and supplements.
- Use medicines as directed and report problems or unexpected effects.
- Keep contact, insurance, and payment information current when relevant.
- Protect medicines from loss, misuse, children, and unauthorized access.
- Treat pharmacy staff and other patients respectfully.
- Ask for clarification when instructions are unclear.
4. Questions, records, and complaints
Contact 8 Mile Rx through its published pharmacy channels with questions, records requests, privacy complaints, medication concerns, or service complaints. When applicable, a HIPAA complaint may also be filed with the U.S. Department of Health and Human Services Office for Civil Rights.
5. Nondiscrimination
8 Mile Rx provides pharmacy services in accordance with applicable civil rights laws. Language assistance, auxiliary aids, and grievance information are available as required.
